MRCEM Notes

Tinea

Key Points

  • Tinea infections are common.
  • Capitis relates to the scalp, cruris the groin, pedis the feet and corporis the body.
  • 1st line treatment is usually topical e.g. imidazole or terbinafne cream
  • Systemic (oral) treatment is appropriate for tinea capitis and onychomycosis (Amorolfine is the best topical treatment for nails but oral terbinafine is superior and advocated by NICE). It may also be used in extensive disease or where topical treatment has failed.
  • Diagnosis is confirmed by nail clippings or skin scrapings. Woods lamp examination can also be useful.
  • Transmission is via close personal contact.
