MRCEM Notes

Knee

The menisci deepen the tibial sockets and act as shock absorbers.

The tibial spine, sometimes referred to as the intercondylar eminence, is the point of attachement for ACL & PCL.

The ACL is the main ligament preventing anterior drawer. The collaterals stabilise varus and valgus movements of the lower limb.

The RCEM syllabus also states candidates should have an appreciation of the way in which the condyles are anatomically adapted for the 'screw home' movement. The 'screw home' mechanism locks the knee when standing which decreases the work of the quadriceps. It happens because the tibia externally rotates 5 degrees in the last 15 degrees of extension, this results in the medial tibial plateau articular surface becoming longer than the lateral tibial plateau.
